Abstract

This paper presents a request form web application for students. The objectives aim to solve document management systems such as system delay, the accuracy of document verification, or human problem, for example, incorrect information or don’t understand words and distorting the meaning of the information. Therefore, they have to submit new documents, which are time-consuming and costly. In this paper, we will solve the problem by developing the request form web application to improve the efficiency of the document system and solve the redundancy while using web applications. Laravel Framework is a tool for developing modern web applications, uses the MVC concept of data design, also known as Model, View, and Controller. It makes the system development process more efficient. There are 34 trial users, and the results are divided into three parts: Part 1: System User Interface satisfactions are 85.29% good to very good, 17.65% moderate. Part 2: Usage side satisfactions are 80.15% good to very good, 19.86% moderate. Part 3: satisfaction system capability is 83.33% good to very good, 14.71% moderate, and 2.94% Fair.
