การพัฒนารูปแบบการมีส่วนร่วมของเภสัชกรในการบริบาลทางเภสัชกรรมผู้ป่วยมะเร็ง โรงพยาบาลเจ้าคุณไพบูลย์พนมทวน

Abstract

บทคัดย่อ

            การวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบการมีส่วนร่วมของเภสัชกรในการให้การบริบาลทางเภสัชกรรมผู้ป่วยมะเร็งที่เข้ารับการรักษาในโรงพยาบาลเจ้าคุณไพบูลย์พนมทวน กลุ่มตัวอย่าง ได้แก่ ผู้ป่วยมะเร็งที่เข้ารับการรักษาในโรงพยาบาล  45 คน แพทย์ 5 คน และพยาบาลวิชาชีพแผนกผู้ป่วยใน 12 คน โดยการคัดเลือกแบบเฉพาะเจาะจง เครื่องมือที่ใช้ในการวิจัยคือ แบบบันทึกข้อมูลผู้ป่วย แบบบันทึกการใช้ยาผู้ป่วยใน และแบบสัมภาษณ์ความพึงพอใจของผู้ป่วยและผู้ให้บริการ เก็บรวบรวมข้อมูลจากเวชระเบียนของผู้ป่วยมะเร็งที่เข้ารับการรักษา ตั้งแต่ กรกฎาคม 2556 ถึง มีนาคม 2557 วิเคราะห์ข้อมูลเชิงปริมาณ โดยใช้ความถี่ ร้อยละ ค่าเฉลี่ย และวิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์ข้อมูลเชิงเนื้อหา

            ผลการวิจัย พบว่า จำนวนปัญหาด้านการใช้ยาเฉลี่ยก่อนและหลังการพัฒนาระบบ เท่ากับ 1.46 ครั้งต่อผู้ป่วย 1 ราย และ 0.47 ครั้งต่อผู้ป่วย 1 ราย ตามลำดับ ด้านความพึงพอใจ พบว่า คะแนนความพึงพอใจของผู้ป่วยที่มีต่อเภสัชกรโดยรวมเฉลี่ยก่อนและหลัง เท่ากับ 3.26 และ 4.12 ตามลำดับ และด้านความพึงพอใจของบุคลากร พบว่า ความพึงพอใจโดยรวมเฉลี่ยก่อนและหลัง เท่ากับ 3.20 และ 4.07 ตามลำดับ

          การวิจัยนี้สรุปได้ว่า การดูแลผู้ป่วยมะเร็งในรูปแบบสหวิชาชีพ โดยการมีส่วนร่วมของเภสัชกร ทำให้สามารถค้นพบปัญหาการใช้ยาและแก้ไขปัญหาได้อย่างเหมาะสม รวมถึงผู้ป่วยและ ผู้ให้บริการมีความพึงพอใจต่อการมีเภสัชกรเข้ามามีส่วนร่วมในการดูแลผู้ป่วย

Abtract

               The purpose of this action research was to develop participation of pharmacist in pharmaceutical care for cancer patients in Jaokunpiboonphanomtuan hospital. The sample consisted of forty-five patients  who admitted in Jaokunpiboonphanomtuan hospital, five physiciants, twelve nurses of in-patient department by purposive sampling. The instruments used included patient data records, patient medication record, satisfaction questionaire of patients and providers. Collected data from medical record of cancer patient who admitted in Jaokunpiboonphanomtuan hospital during July 2556 till March 2557. The study was quantitative and qualitative research. Statistics used to analyze the data were frequency, percentage and average.

               The result of drug  related problem before and after develop system was average 14.6 times per patient and 0.47 times per patient. Patient’s satisfaction before and after develop system was 3.26 and 4.12 points  and provider’s satisfaction before and after develop system was 3.20 and 4.07 points.

               The study showed that care of cancer patients by multidisciplinary team that had pharmacists was found drug related problems  and recommended appropriate medication use. Including,  patients and providers satisfied to participation of pharmacist.
